Notice of Loss Records

Incident: In December 2021, during a routine software upgrade, our external hard drive was damaged, resulting in the loss of clinical records, including additional documents (e.g., medical, school, court records). Unfortunately, these files are unrecoverable. However, the 2021 Financial Reports were successfully retrieved.

No Breach of Confidentiality: We want to assure you that there was no breach of confidentiality or compromise to any client data.

Action Taken: We are working to rebuild the lost files for current clients and may ask for your cooperation in signing new forms. We have partnered with a new HIPAA-compliant PHI company to secure our records moving forward.
